Gold - New PPO Plan this year. Lowest deductible plan with the higher monthly premium. This plan has 80% co insurance after the deductible has been met.

Pharmacy benefit services with BCBSKS will be provided by Express Scripts and administered by RxBenefits. RxBenefits can help you navigate through any issues related to purchasing prescription medication.

Please note:  

The Extended Payment Program (EPP) provides patients with an option to spread out their payments for their medications over three installments using only their credit or debit card. This is a great solution for members who do not want to pay their entire 90-day copayment at once for home delivery order prescriptions. There is no minimum dollar amount required for participation and there is no service fee.

 

A member must actively sign up for EPP through the member web portal or by calling customer service.

Short Term Disability

Accident Insurance

Critical Illness Insurance

When you are enrolled in the Accident and/or Critical Illness plans you may receive a $50 Wellness Benefit for any one covered health screening test per calendar year.  Voya will pay this benefit regardless of the results of the test and if you are enrolled in both plans will pay $50 for the Accident and $50 for the Critical Illness plan.  This benefit is only payable for eligible health screening tests performed as a result of preventative care, which includes some tests and diagnostic procedures ordered in connection with routine examinations.  This benefit is payable for the covered employee and spouse and is available for dependent children at 50% of the employee amount per child, with an annual maximum of $100 for all children.

Indemnity Insurance

If you choose not to enroll in a Blue Cross Blue Shield employer sponsored medical plan. This medical option also includes a minimum essential coverage plan (MEC) which covers all wellness/preventitive services at 100% as mandated by the health care reform. Because this Ternian option includes a MEC plan, it satisfies the "individule mandate" the governments requirement that you have health insurance.
